Registration form

Before you register with the practice, please ensure you live within the practice boundary.  A detailed map of the Practice boundaries is available for inspection.  This boundary was set by Grampian Health Board.

Please bring your NHS medical card to the surgery if possible or you will need to provide a form of identification (passport/driving licence /utility bill etc.). The receptionist will ask you to fill in a registration form and a simple questionnaire which will enable to doctor to assess your medical needs whilst waiting for your past medical records to arrive at the practice.

You will be asked to attend the practice nurse for a medical health check but this is optional.

Change of Personal Details

If you change your name, address or telephone number, please notify the surgery.  It is important that our records are kept up-to-date should we need to communicate with you.  If you or a member of your family move outside the practice boundary, you should register with a doctor in the new locality.
